- Request procedure for access to records
- To enable Lotus Life to process a request for access to information, the requester must complete form C, which is annexed hereto, and submit the form to the Information Officer at his address, fax number or email address provided in paragraph 2.
- The Information Officer will consider the request and notify the requester of his decision in writing, within the time periods stipulated in the Act, indicating whether the request is granted or refused. Should the Information Officer refuse the request, he will advise the requester of the external remedies available to the requester to dispute the decision.
- Fee schedule in terms of Section 54
- A requester, other than a personal requester, must pay a request fee (currently R 50.00) before Lotus Life will process the request. A request fee is an administration fee and is not refundable.
- Once a request for access to information is granted, the requester must pay an access fee to reproduce the requested information. The applicable access fees (currently) are:

Actual postage fee
- Should the preparation of the record for disclosure in the opinion of the Information Officer, require more than the prescribed hours (currently 6 hours), the requester must pay as a deposit the prescribed portion (being not more than one third) of the access fee which would be payable if the request is granted.
- The requester may lodge an application with a court against the payment of the request fee and deposit.
- Lotus Life may withhold access to a record under its control until the requisite fees have been paid.
- Records that cannot be found or do not exist
- Should all reasonable steps have been taken to find a record requested and there are reasonable grounds for believing that the record cannot be found or does not exist, the Information Officer will notify the requester that it is not possible to give access to that record.
- The Information Officer will give a full account of all steps taken to find the record or to determine whether the record exists. If, after notice is given, the record is found, the requester will be given access to the record unless access is refused by the Information Officer.
